Referral
Getting Started
Fill out a referral form and email it back to info@activeconnections.ie Please emphasise what the aims of the intervention will be.
Active Connections CLG (AC) staff will review the referral and assigns a worker.
Our worker will then contact the referrer and arrange a time to hold what we call a 'plan of work meeting' (POW) this meeting is designed to introduce the AC staff member, the programme in full and agree on the aims with the young person family/carers.

Usually, with the young person's agreement, the AC staff member will take the young person on their first session following the POW.

AC will produce session reports following each session but if there are any significant concerns the case manager and family will be notified immediately.
